MEMORIAL BOOK PROGRAM
Remember a friend or loved one, or honor someone on their birthday, anniversary, or baptism through our Memorial Book Program. Please click the link below to print, complete the form and mail to the Bentleyville Public Library at 931 Main Street , Bentleyville, PA 15314.
Memorial donations are also accepted to purchase new items for the Bentleyville Public Library. Your donation is a splendid way to show your support of public libraries, to honor or memorialize a friend or loved one, and to show support of a special event. You may designate the item to be purchased, or suggest a subject area. The Bentleyville Public Library greatly appreciates the many generous donations made to the Memorial Fund. The many delightful and educational books purchased and received through these gifts have enhanced the Library’s collection, and for that we are truly grateful. Guidelines for Memorial Donations:
* We ask $25 to request a title or topic and have a memorial plate placed in the book.
* For donation amounts of $15 and $20, we will select a book and place a memorial plate in it.
* For any amount less than $15 or as designated, we will gratefully accept it as a monetary donation to the Bentleyville Public Library general fund and place a card noting the donation in the Memorial Card drawer.
